PATIENT SUPPORT

What Nobody Warns You
About Aortic Valve Replacement Surgery

The consultant says the words you need “Aortic Valve Replacement”. They talk you through the next steps and move on with their day. But you don’t. That night, you notice your own heartbeat for the first time in years and the worry you carry afterwards is the part almost nobody talks about.

“I just want to feel like myself again.”

You’re not chasing anything dramatic - walk the dog without stopping, get back to work, have a pint with your mates without thinking about your chest once. The thing that pushes most people through our door isn’t fear of surgery. It’s the stubborn longing to have their normal life back.

“What if I’m not around for what matters?”

For patients who have had prior heart surgery, keyhole is frequently the safer option. It avoids the risks associated with re-opening a previously operated chest.

“I’ve stopped trusting my own body.”

Every flutter, every bit of breathlessness, every twinge now comes with a question mark. You find yourself bracing for an episode that may never come - and that constant, low-level vigilance is exhausting. Almost nobody talks about it.

“I can’t face having my chest cracked open.”

For patients who have had prior heart surgery, keyhole is frequently the safer option. It avoids the risks associated with re-opening a previously operated chest.

At The Keyhole Heart Clinic, we’ve listened to all our patients fears and built everything we do around answering them..

From your first consultation, through our minimally invasive keyhole procedure - no broken breastbone, no long recovery - to rehabilitation that can leave you stronger than before, our whole process is designed to give you your life back. Fully. Confidently. Without the fear.

What Is Aortic Valve Disease?

Your aortic valve acts like a door that opens and closes with every heartbeat. When it fails, your heart works harder, and symptoms follow. Understanding your condition is the first step to the right treatment.

Aortic Stenosis

The valve becomes stiff and narrow, forcing your heart to work much harder to push blood through. Left untreated, it leads to heart failure.

Aortic Regurgitation

The valve doesn't close properly, causing blood to leak back. This is often missed until heart enlargement occurs, making careful monitoring essential.

Bicuspid Aortic Valve

A congenital defect where the valve has two leaflets instead of three. It often leads to stenosis or leaking earlier, and can be associated with aortic aneurysms.

Mr. Birdi Explains Keyhole Aortic Valve Replacement

Why Choose Keyhole Aortic Valve Replacement

Traditional aortic valve repair or replacement uses a large chest incision and splits the breastbone.

This incision needs wiring together after the surgery and takes up to 12 weeks to heal, meaning that recovery is prolonged.

Keyhole aortic valve replacement offers several benefits over traditional open-heart surgery through the breastbone.

No broken breastbone

  • Minimal pain

No broken breastbone

Minimal scaring

  • Back to life sooner

Minimal scaring

Back to life sooner

Our most advanced surgical treatment involves no broken bone since we approach the aortic valve through a small cut between the ribs on the right side of the chest. This is called ARTAVR.

This treatment will allow you to recover in a matter of days rather than months and produces less pain, less risk of infection, less bleeding and swifter mobility when performed by experts.

By opting for keyhole aortic valve replacement, performed by experienced teams, patients can enjoy a faster and smoother recovery, while still receiving the same excellent outcome of traditional open-heart surgery.

Keyhole Surgery vs Traditional Open-Heart Surgery

Traditional vs Keyhole Approach Comparison

Keyhole Approach (ARTAVR)

Traditional Open-Heart

Small incision between the ribs with no bone broken
Large cut down the chest with breastbone split apart
Less pain
Increased trauma and pain
Minimal blood loss
Greater blood loss
Lower infection risk
Higher infection risk
Home within 7 days after surgery
Extended hospital stay of often 7+ days
Wound heals in 10 days
12 weeks for the breastbone to heal
Back to normal life in 3 to 4 weeks
Restricted for up to 12 weeks
Particularly safe for elderly & re-do patients
Higher risk for older or previously operated patients

WHAT TO EXPECT

Your Recovery Timeline

Most patients tell us the recovery surprised them. Not because it was hard — but because it was so much easier than they feared. Here is what your journey typically looks like.

DAY OF SURGERY

In Safe Hands

Today is the day. Your procedure is performed by Mr Birdi using a small incision between the ribs — no broken breastbone, no large chest wound. You are under general anaesthetic throughout, and every element of your care has been planned in meticulous detail by a team that does this every week.

Mr Birdi has personally selected every person in that operating theatre — your anaesthetist, your perfusionist, your scrub team — drawing on the best specialist talent from across the country. Cromwell Hospital’s intensive care unit is staffed and ready around the clock. You are in the right place, with the right people.

There is nothing for you to do today except trust the team. That part is taken care of.

DAYS 1–2

One to One Care

You will wake up in the intensive care unit, where you have a dedicated nurse with you at all times. Your pain is carefully managed — most patients are surprised by how comfortable they feel so soon after surgery.

Within the first day, you will sit up and most patients are standing and taking their first steps. This is not remarkable — it is expected. The keyhole approach causes so much less disruption to your body that early movement is not just possible, it is part of the plan.

Your recovery has already begun.

DAYS 3-5

Fine Tuning Your Recovery

You have moved out of intensive care and onto the ward. You are walking further each day, eating normally, and starting to feel like yourself again.

The team are still closely involved — monitoring how your heart is responding, adjusting your medication where needed, and making sure your pain is well controlled. Your physiotherapist will begin working with you on breathing exercises and gentle movement.

This is the phase where most patients stop bracing for difficulty and start to feel quietly confident. The hard part is behind you.

DAYS 5-7

Safe Discharge Home

For most patients, going home happens between days five and seven. Before you leave, your team will go through everything with you in detail — your medication, what to watch for, who to call if you have any concern, and what the coming weeks will look like.

You will not be sent home until you are ready. And once you are home, you are not on your own. Mr Birdi and the team remain contactable. If something worries you — however small — you do not have to wait for your next appointment.

For context: patients recovering from traditional open-heart surgery are typically in hospital for seven days or more. You are already home.

DAYS 7-9

Rapid Wound Healing

Because no bone was broken, your body does not need months to repair itself. The small incision between your ribs — just a few centimetres — heals within seven to nine days for most patients.

You will notice the difference. There is no heavy chest ache from a healing breastbone. No restriction around your ribcage. The wound is small, it heals cleanly, and it leaves a minimal scar.

At this point, most patients cannot quite believe how different this feels from what they were expecting.

WEEK 2

Your Personalised Recovery Plan Commences

From week two, your rehabilitation begins in earnest. Our specialist cardiac rehabilitation partners will work with you on a programme designed specifically around you — your age, your lifestyle, your goals, and the pace that is right for your body.

This is not a generic leaflet. It is a structured, guided plan that builds your strength and stamina week by week, restoring your confidence alongside your fitness. Many patients tell us this phase is where they start to feel genuinely well again — not just recovering, but improving.

Your body has had a serious operation. Your rehabilitation plan makes sure you get the most out of it.

WEEK 4

Getting Your Independence Back

By week four, most patients are driving again, returning to desk-based work, and managing their daily life without assistance. The things that felt distant four weeks ago — a trip to the shops, an evening out, a walk without getting breathless — are back.

This matters. One of the most significant costs of traditional open-heart surgery is not the operation itself — it is the three months of restricted life that follow it. With the keyhole approach, that window shrinks dramatically.

Your consultant will confirm what is right for you individually. But for most patients, week four marks the point where life feels like their own again.

WEEK 6

Back to Your Life

Six weeks after your surgery, you will have your follow-up appointment with Mr Birdi. This is the milestone most patients look forward to — the moment where everything is checked, confirmed, and signed off.

For many, it is also the point where they return to the activities that matter most. Golf. Tennis. The gym. Longer walks. Travel. The things that aortic stenosis had been quietly taking away from them, often for years, are available again.

This is what the surgery was for. Not just to fix a valve — but to give you back the life you had before it started to narrow.

ANNUAL

Your Team for Life

Your relationship with The Keyhole Heart Clinic does not end at your six-week follow-up. For patients who would like ongoing peace of mind, we offer an annual cardiac surveillance programme — a regular check-in with our team to monitor your heart health and make sure everything continues to perform as it should.

This is entirely optional, but many of our patients find it invaluable. Knowing that a specialist eye is reviewing your heart once a year — and that the team who operated on you is still there if anything needs attention — is a form of reassurance that no amount of internet searching can provide.

You chose us for your surgery. We would like to look after your heart for the long term, if you will let us.

Are you Suitable for Keyhole Aortic Valve Replacement?

The great news is that most people who need an aortic valve replacement are suitable for the keyhole approach, particularly when the surgery is performed by an experienced specialist team like ours.

You are likely a good candidate if:

You have been told you need an aortic valve replacement

You are in reasonable general health

Your heart anatomy is suitable (assessed from your scans)

You have had heart surgery before, as keyhole is often safer for re-do operations

You are older, as the gentler approach is particularly well suited to elderly patients

Were you told keyhole isn't an option?

If you were turned away from keyhole surgery elsewhere, it is always worth asking for a second opinion. We have helped many patients who were initially told the keyhole approach wasn't possible for them.

Older or with other health conditions?

The keyhole approach is often especially recommended for older patients. With less blood loss, a shorter hospital stay, and faster mobilisation, it is gentler on the whole body, and age alone is never a barrier.

Suitable for Re-do Heart Surgery

For patients who have had prior heart surgery, keyhole is frequently the safer option. It avoids the risks associated with re-opening a previously operated chest.

OUTCOMES & EXPECTATIONS

How Successful Is Keyhole Aortic Valve Surgery?

Keyhole aortic valve replacement is a well-established and highly effective procedure. Success rates are at least as good as traditional open-heart surgery, and in many patient groups the outcomes are better.

The vast majority of patients notice a significant improvement in how they feel after surgery, including:

Breathlessness and shortness of breath

Energy levels and stamina

Confidence returning to normal activities

Overall quality of life

Many patients find they can once again enjoy activities that their faulty valve had been preventing for months or even years, whether that means walking further, climbing stairs without stopping, keeping up with grandchildren, returning to a hobby, or simply feeling like themselves again.

HIGHLY EXPERIENCED & SKILLED SURGEONS

Why You Should Choose The Keyhole Heart Clinic

When you're facing heart surgery, choosing who performs it is the most important decision you will make. Here is what sets The Keyhole Heart Clinic apart.

A seamless programme from diagnosis to recovery

Designed around one goal: getting your life back

Most patients who need heart surgery encounter a fragmented system. A cardiologist here, a surgeon there, a GP in between, and rehabilitation that is either generic or nonexistent. At The Keyhole Heart Clinic, every step is joined up. From your first assessment through to post-operative follow-up and cardiac rehabilitation, you move through a single, coordinated programme led by the same team, with one purpose. Not just to fix your valve. To get you back to your life as quickly as humanly possible.

One of only a handful of surgeons in the world with this skillset

Keyhole aortic valve replacement is not performed routinely across the UK. It demands a highly specialised technique that takes decades to master. Mr Birdi is recognised internationally as one of only a small number of surgeons anywhere in the world capable of performing complex keyhole cardiac procedures, including keyhole bypass and keyhole aortic valve repair combined. Many of our patients come to us having been told elsewhere that keyhole wasn't possible for them. We regularly find that it is.

More than 2,000 minimally invasive procedures performed personally

Volume matters in surgery. The more procedures a surgeon has performed, the lower the complication rate and the better the outcome. Mr Birdi has personally completed over 2,000 minimally invasive heart procedures and close to 8,000 total cardiac operations throughout his career. This depth of experience is rare. It means that whatever your anatomy, whatever your history, he is likely to have encountered it before.

UK and world firsts

Designed around one goal: getting your life back

In 2020, Mr Birdi performed the UK's first combined keyhole aortic and mitral valve repair in a single procedure. That same year he pioneered a world-first: a minimally invasive operation combining valve surgery and coronary bypass, previously only ever attempted as two separate operations. In 2022, he performed the UK's first quadruple keyhole heart bypass. These are not incremental improvements. They are landmarks that have changed what is considered possible in cardiac surgery.

You are seen, assessed and operated on by the same surgeon

At The Keyhole Heart Clinic, Mr Birdi reviews every patient personally. Every scan, every blood result, every surgical decision. You will not be assessed by a registrar and operated on by someone you have never met. The surgeon who sits across from you in consultation is the surgeon who will be with you in theatre. That continuity of care is something most patients tell us they had not expected, and value enormously.

A handpicked expert team at one of the UK's leading hospitals

Surgery is never a solo performance. Your anaesthetist, perfusionist, scrub team, and intensive care nurses are all personally selected by Mr Birdi for their expertise in complex cardiac cases. Operations take place at Cromwell Hospital in central London, one of the UK's leading private hospitals, with a fully equipped 24-hour intensive care unit and specialist cardiac staff present around the clock. You are never alone in your recovery.

FREQUENTLY ASKED QUESTIONS

Aortic Valve Replacement

What is keyhole aortic valve replacement and what does the operation involve?

Keyhole aortic valve replacement (ARTAVR) replaces your faulty aortic valve through a small incision between the ribs, no broken breastbone. The surgical result is identical to traditional surgery, but because no bone is cut, the wound heals in around 7 to 10 days and most patients are home within 3 to 5 days.

What is the difference between keyhole surgery and traditional open-heart surgery?

Traditional surgery splits the breastbone open, taking around 12 weeks to heal with significant restrictions on daily life. Keyhole surgery uses a small side incision, no bone broken, meaning less pain, less blood loss, lower infection risk, and a return to normal life in 3 to 4 weeks rather than 3 months.

Am I suitable for keyhole aortic valve surgery?

Most patients who need an aortic valve replacement are suitable for the keyhole approach, including older patients and those who have had previous heart surgery. If you have been told elsewhere that keyhole is not an option, it is always worth asking for a second opinion.

How long does recovery take after keyhole aortic valve surgery?

Most patients are home within 3 to 5 days, driving within 4 weeks, and back to hobbies and normal daily life within 4 to 6 weeks. By contrast, traditional open-heart surgery requires roughly 12 weeks before the breastbone heals.

Is keyhole surgery officially recommended and is it safe?

Yes. NICE published guidelines in 2021 recommending that suitable patients be referred to a specialist centre — The Keyhole Heart Clinic is one of those centres. Mr Birdi has performed over 2,000 minimally invasive heart procedures, and your surgery takes place at Cromwell Hospital with a fully staffed 24-hour ICU..

Will my health insurance cover it and what does a consultation cost?

In most cases, yes, keyhole aortic valve replacement is covered by all major UK private health insurers including BUPA, Vitality, Aviva, Cigna, and WPA. If you are self-paying, a face-to-face consultation is £450 and an online consultation is £325.
